Agreement to Terms & Liability Waiver

 

*PLEASE READ CAREFULLY BEFORE SIGNING UP. THIS IS A LEGAL DOCUMENT.

 

1. Code of Conduct: I agree to abide by the OHM Sole Proprietorship t/a Lineup Club Code of Conduct, which promotes good sportsmanship, respect for players, staff, equipment, and venues. I understand that failure to do so may result in my removal from events without a refund.

 

2. Liability Waiver, Release, and Assumption of Risk: I acknowledge that participating in pool tournaments and related activities involves inherent risks, including but not limited to physical injury.

 

In consideration of being permitted to participate in any OHM Sole Proprietorship t/a Lineup Club event, I, the undersigned, hereby:

*Waive and Release OHM Sole Proprietorship t/a Lineup Club, its agents, employees, sponsors (including United King Pool Tables), subsidiaries and host venues from any and all liability, claims, demands, or causes of action for any loss, injury, damage, or expense that I may suffer, arising from my participation in these activities, including travel to and from events.

 

*Assume all Risks inherent to such participation, whether known or unknown.

 

*Indemnify and Hold Harmless the released parties from any and all liability for any property damage or personal injury to any third party resulting from my participation.

 

I confirm that I am in good health and physically able to participate. I have read and fully understand this waiver and release of liability.

 

*Photo/Video Release (Optional): I grant OHM Sole Proprietorship t/a Lineup Club permission to use any photographs or video footage taken of me during events for promotional purposes (e.g., website, social media, or any other form of media that may be used for promoting Lineup Club).

 

 

*Please note that hard copies will be made available for initial confirmation on first arrival to the Lineup Club, and will be filed in accordance to the initiates portfolio, as a registered member of the Lineup Club.
